    General Information

    Co-Pays: Co-payments are a contractual obligation with your insurance company. You are required to pay your co-payments, and we are required to collect your co-payments at the time of each visit. For your convenience, we accept Visa, MasterCard, cash, and checks.

    Appointment Cancellations: Appointments not cancelled within at least 24 hours in advance will be charged a $50 fee ($100 for cosmetic appointments).

    New Patient Forms

    When you come for your first appointment, we ask that you arrive 10-15 minutes early to fill out our new patient forms. Please also remember to bring your insurance card with you.

    For your convenience and to save time, you can download and complete the new patient forms, email a copy and bring them with you.

    IMPORTANT!

    1. DOWNLOAD THE FREE ADOBE ACROBAT READER (link)
    2. DOWNLOAD  THE FOLLOWING 2 FORMS AND EXTRACT THEM TO YOUR HARD DRIVE USING AN EXTRACTION PROGRAM LIKE WINZIP
    3. FILL THEM OUT AND SUBMIT VIA EMAIL. The submit button at the bottom of each form will automatically fill in the email address to our front office.
